Dr. Alan J. Vallarine

President-CEO

Dr. Vallarine graduated from Loyola Marymount University in 1977 and went on to study at the University of Southern California, School of Dentistry. Upon graduating from USC in 1981, he opened his first dental practice in Turlock, California. Over the past four decades, Dr. Vallarine’ s practices have grown and progressed, but they have never lost their personal feel or sense of values. He understands the importance of providing each and every patient with the personal attention they deserve and takes pride in his practice's uncompromising commitment to providing quality comprehensive patient care. Dr. Vallarine is dedicated to continuously exceeding his patient's expectations and has ensured that each of his offices provides the most up-to-date technologies and procedures, as well as a highly trained and experienced staff. He is a member of the American Dental Association, the California Dental Association and the Stanislaus County Dental Association. In his free time, he enjoys traveling, fishing, and spending time with his family. Dr. Vallarine retired from Clinical Dentistry in 2010 and has focused his energy on managing Fresno Dental Surgery Center.
Dr. Vahaken Arslanian

Dental Director

Dr. Vahaken Arslanian received his B.S. in chemistry from Cal State Northridge in 1989. He continued his education at Loma Linda University where he earned his Doctor of Dental Surgery in 1993. He joined the Loma Linda University school of Dentistry faculty soon after graduation and maintained a private practice in Southern California. In 1997 he moved to the Central Valley where he has been rendering general dentistry services for children under anesthesia at multiple outpatient surgery centers. Dr. Arslanian has been part of the Fresno Dental Surgery Center team since 2007. When he isn’t at the center, he enjoys spending time with his family.
Dr. Paul Ayson

Dentist

Dr. Paul Ayson is General Dentist that has been in practice since 1999. He limits his practice to children. He graduated from the University of Washington with a baccalaureate degree in Zoology in 1993. He then received his DDS degree from the Arthur A. Dugoni School of Dentistry in 1999. After earning his DDS, Dr. Ayson served in the US Air Force from 1999 to 2002, stationed at LaughlinAFB, TX. After active-duty service, Dr. Ayson worked at a Federally Qualified Health Center for 7 years, as well as working in a pediatric dental practice in Salinas, CA. Working at the pediatric dental practice in Salinas is where Dr. Ayson found his niche. In 2009, Dr. Ayson, earned an MBA with an emphasis in Healthcare Management; he also earned his graduate certificates in Healthcare Ethics from the University of Missouri and Healthcare Law from Seton Hall University.  In addition to treating patients at FDSC, Dr. Ayson is also a supervising dentist at the Fresno City College Dental Hygiene Program. Furthermore, he is very active in organized dentistry. He has served on the board of Tulare Kings Dental Society, and is currently a member of the Fresno Madera Dental Society Board. Presently, Dr. Ayson serves on the CDA Government Affairs Council, the Dental Hygiene Review Committee of the American Dental Association's Commission on Dental Accreditation and a Board Member of the Arthur A. Dugoni School of Dentistry Alumni Association. On his spare time, he spends it with his family, rides his bicycle and is an avid college football fan.
Dr. Richard Spencer

Dentist

Dr. Richard Spencer graduated from The University of the Pacific in 1972 with a B.A. in psychology. He received his Doctor of Dental Surgery from the University of the Pacific School of Dentistry in 1976 and completed a hospital based General Practice Dental Residency at Valley Medical Center, Fresno, California in 1977. He was engaged in the private practice of general dentistry from 1977 to 2016. While in private practice, Dr. Spencer dedicated a significant part of his time to the treatment of developmentally and medically compromised children; and maintained staff privileges at Children’s Hospital Central California, Madera and Kaiser Permanente Hospital, Fresno. Since leaving private practice, Dr. Spencer has continued to provide restorative dental services to the children of the central valley through Fresno Dental Surgery Center. He enjoys spending spare time with his family and remains inspired by the unique energy, imagination and enthusiasm exhibited by each of the young patients he is privileged to care for. 
Robert Orr, CRNA, MBA

Anesthesia Director

Robert Orr started his career in medicine at an early age. In high school, he became a First Responder working with the Civil Defense Explorer Post. He then joined the US Navy Reserves as a Navy Corpsman while in high school in 1986. He later transferred to the Mississippi National Guard as a combat medic to continue his service while he attended the University of Southern Mississippi.  Robert earned a B.S.N. at the University of Southern Mississippi in 1991 with a minor in Military Science while commissioned in the US Army through the R.O.T.C. program. Robert graduated from Xavier University/Charity Hospital of New Orleans in 1996 with an M.S. in Nurse Anesthesiology. In 2006, he retired from the US Army Reserves and moved to Clovis, California in the Central Valley. Robert has worked with the Omni Anesthesia group covering such facilities as Eye-Q Vision Center, Herndon Surgery Center, Community Regional Medical Center, Pristine Surgical Center, and Fresno Dental Surgery Center. Robert started working at Fresno Dental Surgery Center in 2008 and later became the Director of Anesthesia there in 2012.  He has been closely working with the dentists, nurses and other staff members to provide a safe environment for the outpatient center. In 2013 while working full-time, Robert completed an M.B.A. from the University of Phoenix. In recent years Robert transitioned to the Central California Anesthesiology Solutions group which absorbed most of the Omni Anesthesia group contracts. Now Robert, while being the Director of Anesthesia, also works at Eye-Q Vision Center, Renaissance Surgery Center, Women’s Plastics and several Oral Maxillofacial Surgery Offices in Fresno. In February 2023, Robert published an article in the American Association of Nurse Anesthetist Journal, “Full Mouth Dental Restoration on a Treacher-Collins Patient without Intubation.”  Fresno Dental Surgery Center continues to provide excellent care for lots of special needs patients.
Dr. Elizabeth Cudilo

Anesthesiologist

Double-boarded in anesthesiology and pediatric anesthesiology, Dr. Elizabeth Cudilo joined Fresno Dental Surgery Center in 2024. She's passionate about multi-modal pain care, taking care of complex pain patients, and serving patients with special needs—especially patients with autism spectrum disorders via the George's Pass Program. Dr. Cudilo is fluent in Polish and speaks some Spanish. In her free time, she enjoys spending time with her family, playing Pokémon GO with her kids, exploring Central California, and cooking.
Dr. Marek Domanski

Anesthesiologist

Board certified by the American Board of Anesthesiology; Dr. Marek Domanski joined Fresno Dental Surgery Center in 2025 as a pediatric anesthesiologist. He has more than 20 years’ experience as a physician and has worked at Phelps County Regional Medical Center in Rolla, Mo., where he served as a staff anesthesiologist. Dr. Domanski received his medical degree from the Pomeranian School of Medicine in Szczecin, Poland. He completed a residency in anesthesiology at New York Methodist Hospital in Brooklyn, N.Y. He completed his fellowship in pediatric anesthesiology at University of Michigan, C.S. Mott Children’s Hospital, in Ann Arbor, Mich. Dr. Domanski is a member of several professional organizations, including the American Society of Anesthesiologists. 
Dr. Sri Smitha Kanaparthy

Anesthesiologist

Dr. Smitha Kanaparthy graduated from the Jawaharlal Institute of Postgraduate Medical Education and Research (JIPMER) in Puducherry, India. completed a residency in anesthesiology and critical care at JIPMER. she worked at very well-known tertiary care centers in India. She came to the US in 2008 and completed her residency in anesthesiology at Hahnemann University Hospital, Drexel University, Philadelphia pediatric anesthesiology fellowship at the Oklahoma University Health Sciences Center. Her work experience after fellowship spanned academic centers at OUHSC and Yale University and a private practice group at Valley Children’s Hospital, Fresno. She is board-certified in anesthesiology and pediatric anesthesiology. She has over ten years of experience exclusively as a pediatric anesthesiologist. Her areas of interest are pediatric difficult airways, anesthesia in non-OR settings, and resident education. She believes in lifelong learning and the integral role of physicians in clinical care and leadership. has completed a faculty leadership program at OUHSC and programs with the Yale School of Business—Health Care Management and Women’s Leadership at Oxford School of Business. She enjoys providing much-needed, safe, and evidence-based care to pediatric patients for dental care in office-based settings. When she is not working, she enjoys spending time with her husband and daughter. They enjoy traveling and exploring beautiful California.
Dr. Richard Katz

Anesthesiologist

Dr. Richard Katz graduated from Albert Einstein College of Medicine, completed his residency in Anesthesiology at University of California Los Angeles (UCLA)., with his Pediatric subspecialty training both at UCLA and Cedars Sinai Medical Center. He is board certified in Anesthesiology. Dr. Katz has been in private practice for more than 30 years, has taken care of thousands of patients and has held many department chairman and executive positions with Associated Anesthesiologists Medical Group, Medical Anesthesia Consultants, Sheridan Anesthesia Service and Envision Physician Services. While at Envision, he served as a Regional Medical Director of Anesthesia for California , where he oversaw and helped develop the anesthesia practice at over 15 hospitals and 30 surgery centers. Dr. Katz was also an Adjunct Clinical Instructor at Stanford and is a General Anesthesia Evaluator for the Dental Board of California. He co-founded Bay Area Mobile Anesthesia with Dr. Vu Huynh in 2011. After 30 years as a hospital-based Anesthesiologist, Dr. Katz now exclusively works with Fresno Dental Surgery Center seeing only dental patients. He also does consulting work for medical practices, dental practices and surgical facilities. When not working, he enjoys spending time with his family, hiking the hills and road trips in the Western US.
Dr. Norah Mazid

Anesthesiologist

Dr. Mazid graduated from Touro University California and completed her residency at Mount Sinai St. Luke’s Roosevelt Hospital Center in New York City. Following her residency, she pursued a fellowship in Pediatric Anesthesiology at St. Christopher’s Hospital for Children in Philadelphia. Dr. Mazid holds board certifications in both Anesthesiology and Pediatric Anesthesiology.
Before joining Fresno Dental Surgery Center in 2024, Dr. Mazid dedicated over five years of her career to Valley Children’s Hospital in Madera where she also served as Department Chairman during her last two years. Her commitment to the medical field began during her undergraduate years at UC Berkeley where she volunteered at a hospital. Her passion for pediatric anesthesiology grew significantly through her involvement in the Operation Rainbow medical mission program, where she provided care for children in need of orthopedic surgeries in Honduras and Nicaragua. Outside of her professional endeavors, Dr. Mazid values spending quality time with her husband and two young daughters.
Samuel Benavidez, RN

Director of Clinical Services and Infection Control Director

Sam is a military veteran and registered nurse serving as the Director of Nursing and Infection Control Director at Fresno Dental Surgery Center.  With over 20+ years of experience in the medical field as a Navy Corpsman and a pediatric nurse. He was drawn to nursing in his passion to help patients understand their health needs. He loves working with children and helping them feel comfortable before any treatment. Certifications that he has obtained include base life support, advanced cardiovascular life support, and pediatric advanced life support. Sam leads the clinical team, ensuring high-quality patient care and safety. Sam is happily married with 2 children. When Sam isn’t at work you can find him cheering on the Raiders football team or spending time with his family.
